Certified Independent Self Direction Broker and Special Needs Consultant
Welcome! My name is Cicely, and I bring 20 years of experience in the special needs field, including nearly a decade as a Medicaid Service Coordinator in New York City. During my tenure, I supported individuals and families navigating complex systems to access critical services. Since the transition to conflict-free case management in 2018, I have been working as an Independent Broker and Special Needs Consultant.
At Envisioned Lives, my mission is to guide parents, caregivers, and self-advocates on their unique journeys. As an Independent Broker, I partner with multiple Fiscal Intermediaries and the Circle of Support Team to design and implement individualized Self-Direction Budgets.
What I Offer:

Comprehensive Support and Organizational Coaching: Beyond Self-Direction, I guide families on how to effectively interact with care coordinators, access services and supports outside the self-direction budget, and understand their Life Plan. I also provide insights and suggestions to ensure it aligns with their goals. Additionally, I explain the HCBS Medicaid Waiver, its importance, and how to keep it active. My personalized one-on-one and group coaching sessions are designed to help families stay organized, informed, and confident as they navigate the complexities of available services. Start-Up Brokerage Services: I assist with creating your initial Self-Direction Budget, participate in the launch meeting, and ensure a smooth transition into self-directed services. (Currently not accepting new clients)

Self Direction
​As an Independent Start-Up and Support Broker, I assist children and adults with intellectual and developmental disabilities who are approved by OPWDD in New York State to self-direct their services. I facilitate the development of the self-direction budget that complies with Medicaid standards. Once support brokerage begins, I support and educate the individual and their family in managing the budget and ensuring Medicaid and State funded services are used following the guidelines of OPWDD.
Organizational Coaching
​Envisioned Lives is committed to empowering the special needs and neurodiverse community by offering personalized one-on-one coaching services. We assist in compiling and organizing information to help you understand and access your child's service needs effectively. Our goal is to guide and navigate parents toward the right resources and services for their child, providing each family with a tailored action plan that ensures they can advocate for and secure the necessary support.
